How to Request PSA-Issued (NSO) Death Certificate online via PSA Serbilis

The PSA Serbilis website is PSA's online facility for submitting requests for copy issuance of civil registry records (birth, marriage and death certificates) and issuance of certification of no marriage record (CENOMAR). PSA is formerly the National Statistic Office or NSO.  (Source)

Below is the step-by-step procedure to order a PSA-issued Death Certificate

STEP 1. Fill-up online information to request a Death Certificate

1. Go to PSA Serbilis website to request for a PSA-issued Death Certificate and other certificates (Birth, Marriage, and CENOMAR). You have 3 options to click on where to start:

  • You can press the Click here to request now button
  • You can click the Step 1 button. A new window will open and press the Click here to request now button
  • You can go to Request tab and select Online Application. A new window will open and press the Click here to request now button

 

2. Read and understand the PSA Serbilis Terms and Conditions. To agree with the condition stated click the I Accept button to proceed. 

3. Enter your name in Requester’s Name and the complete Delivery Address  on where the document will be sent. Then, enter your Telephone or Mobile Number to enable the PSA Serbilis to contact you if required. Also, enter your valid Email Address to receive the payment and delivery notifications. 

Click the Next button to continue.

4. Double-check the details that you have entered and read the consent confirmation. Then, press the Confirm button for the next step. If there are any changes, you can click the Change/Edit button to edit the provided information.

5. This is the Request Summary form. You will notice that the Contact and Delivery Information is shown in the right side of the form. You can still edit the details anytime for any changes.

To request for a Death Certificate, click the Death Certificate button.

6. Answer the following questions accurately.

  • Was there any correction or amendment done to the birth certificate you are applying for?
  • Were you able to previously get a copy of the birth certificate from PSA?

7. Fill-up the following information to request for a Marriage Certificate:

    • Number of copies you are requesting
    • Name of the deceased
    • Sex
    • Place of death
    • Date of death
    • Date of registration, if the death certificate was late registered
    • Purpose of your request

Then, click the Save button to continue.

8. Make sure that the information you have entered in each field are correct. Once all details are checked, click Confirm button to proceed. 

9. Your request will now appear in the Requests Summary same as below. If you want to edit any information in your request, you can click the edit button found in the last column of the form.

If you want to request for another certificate, click the specific button to add new request. 

Once you have completed your request, click the Submit button. 

10. You will receive an acknowledgement once your request has been submitted. A Batch Request Number is generated in every application, while each request will be issued a Request Reference Number. This reference number will be used for paying the said request. When paying for all requests in the batch, specify the Batch Request Number. When paying for a particular request only, specify the Request Reference Number. 

This acknowledgement will be sent also to your email address that you have provided in the contact information.

STEP 2. Pay through accredited payment channels

There are accredited payment channels for paying for your request either by online payment or over the counter. Available payment channels also depend on the delivery address, whether it is within the Philippines or abroad.

If the delivery address is within the Philippines, you can pay via online payment channel using Credit Card/Debit Card, BDO Online Banking, Union Bank Online Banking, and Bayad Center mobile app. While, for over the counter, you can pay in any BDO and Union Bank branches, and in any Bayad Center.

If the delivery address is outside of the Philippines, payment channels are via Credit Card, BDO Remit and Foreign partner banks.

Pay Online via Bayad Center Mobile App

To pay your request via Bayad Center Mobile App, open your account, tap the Pay Bills icon and select PSA Serbilis (Unisys) under Government category. 

Over-the Counter:

You can pay your request in any Bayad Center, BDO and Union Bank branches. Make sure that you have a print out of your request in two copies. 

BDO Online Banking:

If you have a BDO account, you can open your BDO Online Banking and pay your request using Pay Bills.

Union Bank Online Banking:

If you have a Union Bank account, you can open your Union Bank Online Banking and pay your request using Pay Bills.

STEP 3. Wait for delivery

The requested certificate will be delivered by PSA Serbilis through their courier service provider. You will receive the document in 3-13 days after payment, depending on the delivery address you have given in your request.

How to check the status of your PSA Serbilis Request?

To check the status of your request, go to the Home Page of PSA Serbilis. Look for the Check the Status of your Request tab. Enter the Batch Request Number or Request Reference Number and click the Go button.

The status of your request will appear. The description of the result will be displayed based on the current status.

What is the difference between the BATCH REQUEST NUMBER and the REQUEST REFERENCE NUMBER?

Batch Request Number is the generated number for ALL your requests in the batch while Request Reference Number is the generated number in each certificate you have requested. 

For payment, to pay for ALL your requests, use the Batch Request Number. To pay for a PARTICULAR or a specific certificate, use the Request Reference Number.
