  1. To start, you need to complete the form as below. First, select the Examination Name (e.g. Certified Public Accountant). Once you have selected the examination name, you can choose the Examination Date from the drop down menu option when did you take the license examination. Next, type-in your Application Number; this number is the number indicated in your Notice of Admission (NOA) or Licensure Examination Application Form. Then, type your First Name, Last Name, and Birth Date. Once all information is entered, click the VERIFY button to know your grades, if there is any record.
