No SS Number yet? Apply online!

For individuals who do not yet have an SS number, SSS has introduced the online application through the SSS website. Applicants must have an email address required for the confirmation of the registration process link. Then, a series of forms will be filled out to enter the member’s personal information, contact number, address, and other relevant details.

For the purpose of the registration, there are four options to choose from:

    • For Employment
    • Self Employed
    • Overseas Filipino Worker
    • Non Working Spouse

After completing the information required for registration, an SS Number will be generated along with the ePersonal Record Form and SS Number Slip. You must print these documents and proceed to any nearest SSS branch to complete the registration process and apply for a UMID card.

  1. Birth Certificate
  2. In the absence of the Birth Certificate, any of the following documents:
    • Baptismal Certificate
    • Driver’s License
    • Passport
    • Professional Regulation Commission (PRC) card
    • Seaman’s Book(Seafarer’s Identification and Record Book)
  3. In the absence of the above ID card(s) and/or documents, any two (2) of the following, both with the correct name and at least one(1) with date of birth:
    • (GSIS) card/Member’s Record/Certificate of Membership
    • ATM Card (with cardholder’s name)
    • Alien Certificate of Registration
    • Bank Account Passbook
    • Baptismal Certificate of child/ren
    • Birth Certificate of child/ren
    • Cert. of Licensure/Qualification Document from Maritime Industry Auth.
    • Certificate of Confirmation issued by National Commission on Indigenous Peoples (formerly Office of Southern Cultural Community and Office of Northern Cultural Community
    • Certificate of Muslim Filipino Tribal Affiliation issued by National Commission on Muslim Filipinos
    • Certificate of Non-Availability of Birth Records from the City or Municipal Civil Registrar or Philippine Statistic Authority/National Statistics Office or National Archives, for the alleged correct name/date of birth
    • Company ID card
    • Court Order granting petition for change of name or date of birth
    • Court Order granting petition for correction of sex, if with erroneous entry of sex in Birth Certificate
    • Credit card
    • Firearm License card issued by Philippine National Police (PNP)
    • Fishworker’s License issued by Bureau of Fisheries and Aquatic Resources (BFAR)
    • Health or Medical Card
    • Home Development Mutual Fund (Pag-IBIG) Member’s Data Form/Transaction Card
    • Homeowners Association ID Card
    • ID card issued by (LGUs) (e.g Barangay/Municipality/City)
    • ID card issued by professional association recognized by PRC
    • Life Insurance Policy
    • Marriage Contract/Certificate of Marriage
    • Member’s copy of Personal Record(SS Form E-1, RS-1, OW-1, NW-1) duly received by the SSS where the correct sex is indicated
    • Membership card issued by Private Co.
    • National Bureau of Investigation (NBI) Clearance
    • Overseas Worker Welfare Administration (OWWA) card
    • Philippine Health Insurance Corporation (PHIC) ID card/Member Data Record
    • Police Clearance
    • Postal ID card
    • School ID card
    • Seafarer’s Registration Certificate issued by Philippine Overseas Employment Administration (POEA)
    • Senior Citizen card
    • Student Permit issued by Land Transportation Office (LTO)
    • Taxpayer’s Identification Number (TIN) card
    • Transcript of Records
    • Voter’s Identification card or Affidavit/Certificate of Registration

The registrant’s membership status with SSS shall remain “Temporary” until the required supporting documents have been submitted to the nearest SSS Branch.

Submission of required documents will convert the registrant’s membership status with SSS from “Temporary” to “Permanent“.

Availment of loans and benefits (subject to qualifying conditions) and UMID card application (with at least one posted contribution) will be allowed only for “Permanent” membership status.

1 Go to this SSS Website link and press the click here button.

A new page will open, https://member.sss.gov.ph/members/rcsmi/main.html. Read the step by step guide and click the Start button.

2 Complete the information needed for Registrant Record Verification. Then tick the box for security and certify that the details you have provided are true and correct. And, press the Submit button.

3 A confirmation link will be sent to your email. Once received, click the link to continue with your SS Number application.

4 Enter the required details in each form:

  • Basic Information
  • Contact Information
  • Home Address
  • Purpose of Registration

and click the Next button.

5 Then, enter your Social Status and Place of Birth, and press the Next button.

 

6 For Beneficiary(ies), if there is any, input their details and click the Next button to proceed.

7 Review the details and make sure that all information is correct. If there is any error in your entries, click the Previous button to edit it. 

If all information is correct, click the Generate SS Number button and wait for the system to display the generated SS Number and an option to print the ePersonal Record Form and SS Number Slip.

An email will be sent for further instructions.

8 Print a copy of the generated ePersonal Record Form, SS Number Slip, and the SS Number Application Confirmation email body sent to you by the SS Number Issuance System.

9 Go to the nearest SSS Branch and submit your printed records together with the required documentary requirements.

  • If married, bring a copy of your Marriage Contract/Certificate
  • If with child(ren), also bring a copy(ies) of the Birth Certificate(s)
  • For Non-Working Spouse, your working spouse must sign before his/her name on the printed ePersonal Record Form. The signature will signify that your spouse agrees with your SSS membership.

      Just follow the steps on this guide and ypu will be able to start application for your son SSS. However, your son needs to go SSS branch for picture taking for the ID. Is your son 18 years old or older?
